Share what matters most

The one thing you most need the clinician to hear today — in the fewest words possible.

Example

Mom hasn't slept more than 2 hours at a time for the past 5 nights, and she's confused in the mornings.

Their recent history

Current medications, conditions, and what changed in the last few days or weeks.

Example

Taking regular medications as usual but started a new antibiotic Friday. Has had two falls this week.

About what I think

Your honest concern, in one short sentence — what you suspect or fear may be going on.

Example

I think the new medication might be making her dizzy, and I'm worried she's heading for another fall.

Request

My care partner needs… how will we do this? Be clear about what you need: a med review, referral, call, or written plan.

Example

Can we review her meds, tell me how the infection will be tested, and make a plan for what to do if she falls again at home?

T

Track it

Write down what you were told — names, doses, dates, who to call, and the next appointment.

Example

Stop current antibiotic, start new one tomorrow with food. Recheck in 1 week. Call Dr. Patel at 555-0142 if another fall. Follow-up May 15.
